Labeling your kayak

I searched for these labels a couple times, and have seen pictures of them, I was at first under the impression that all the info you send will be printed on the sticker, I'm not too sure about that , they might be blank labels that you write on. Blank labels shouldn't need that much contact info though. Couple searches found that you can pick them up at some tackle shops, how would they print them?......Jack
